What our field signals tell us about that failure is that it isn’t a tool problem, not even a code-related issue.

In fact, AI-powered tools used in software development are getting more complete and precise, fast. The most advanced, like Claude Code, are even designed to act without waiting for human approval.

The main issues come from what’s around code generation: system architecture & engineering practices.

Code Validation

One of the main trade-offs of using AI tools to code faster is the increasing review time. At least in theory. Because at this step, too, AI is tricky.

With the increasing use of AI-generated code, a new pattern is emerging: “vibe-based validation. This marks a shift where engineers review LLM outputs, see that they look clean and syntactically well-written, and validate them.

This illusion of correctness is inherent to LLMs. They’re optimized for plausibility, producing outputs that are syntactically correct and internally consistent, even when the logic is flawed.

In traditional development, these errors are caught because the author has built a mental map of the system. With AI, that process is reversed: engineers are forced to validate a snapshot of a map they didn't draw.

Over time, the engineering team loses its deep system literacy, replacing a comprehensive mental map with a series of vibe-based, validated snapshots. Later on, that loss of deep system literacy complicates debugging.

Ultimately, while AI tools can write functions fast, they create a cognitive bypass that generates a false sense of control and leads to a superficial Human-in-the-Loop (HITL) approach, compromising the entire system in the long run.

For leadership teams, prompting code without senior oversight is a waste of resources and generates long-term liability.

System behavior

AI lacks the mental map of systems’ technical debt, scaling bottlenecks, and unique architecture choices.

While an LLM can provide a syntactically perfect function, it can’t account for the reasoning behind systems’ design.

In this case, the cost of velocity is higher system complexity. AI provides a statistically probable answer, but it’s not aligned with the specific legacy environment.

When AI-generated code bypasses established patterns, technical debt accumulates rapidly. Ultimately, the system evolves faster than the AI engineering team can comprehend and document, creating a legacy system in real time.

Debugging

When a software engineer crafts a complex module, they build an internal mental map of the reasoning, edge cases, and dependencies involved. That "why" is the foundation of rapid troubleshooting.

When AI generates the logic, that mental map doesn’t exist. When bugs inevitably appear, engineers are forced to perform reverse engineering on the AI’s output just to understand the logic before they can even begin to fix them.

This creates a paradox: while the time to code drops, the Mean Time to Recovery (MTTR) spikes. The time saved during initial development is quickly swallowed by the hidden costs of maintaining a system that no one on the team truly understands.

Ownership

While headlines suggest that AI threatens the future of software engineers, the reality is that their expertise is even more critical to achieve successful AI adoption in software.

Coding is no longer scarce. Senior engineering judgement is.

The current failure on the field stems from the diffuse responsibility. We see a widening gap in ownership on two levels:

  • Outcome Ownership: The accountability for the system’s behavior, security, and long-term stability, regardless of the code “author”.
  • Strategic Ownership: The high-level decision-making regarding which processes should be automated or not.

Without accountability, speed is a liability. Since AI has removed the bottleneck at the code-writing level, the new priority for AI engineering teams is providing the oversight needed to ensure the system architecture can absorb high-velocity AI output without collapsing.

Deep System Literacy: From Authoring to Auditing

When engineering teams understand the “why” behind the code, AI governance becomes proactive. It’s crucial in the AI era, as engineers’ primary responsibility shifts from “authoring syntax” to auditing AI outputs against the company’s safety and performance guardrails.
